Far as I know... most folks don't use the head end ones. So they aren't put on.
They are on both sides so you don't have to pick right or left lay.
Would a knotty near the head be good for keeping your pillow from falling out of the hammock?

I did a head and a foot knotty on opposing sides. I like the way it creates a near vertical wall near my feet and head. I also incorporated "d" rings at the shock cord tie off points to use as side tie outs if I wanted. I think it really comes down to what you're looking for in your hammock. That's the fun of DIY! There is no wrong way to do something if it works for you.

Yes, that's exactly how it is on my SnrMoment DIY. Keeps the pillow in, keeps the end of the the TQ in. Ran into the latter this last trip out which reminded me why I liked the mod... trying out a 58" wide Dutch hammock and my feet kept overshooting the edge.
I've never had a problem with the head end, but I can see where some might be annoyed by it, depending on placement I guess?

I put a head end and a foot end for my hammocks. I really like it. Creates a modicum of foot box / shoulder box. I tried doing both on the foot end, but It just didn't have the same feel.
