Does it have any cams with it, and does it matter whether the bobbin is vertical or horizontal? I've got an old 60's Japanese ZIg Zag clone that this might work on.
Also, is there a feed dog? I can't tell from the picture.
Looks to me like cams are shown in the pic.
The box identifies it being for the slant needle design. I would think that is more important than the bobbin orientation. I would be hesitant to try it on a straight needle machine. Usually those accessories work as free standing units. The feed dogs on the machine are lowered and the fabric is clamped in the frame of the buttonholer. The bottonholer moves the fabric according to the cam design.

Bobbin Tension - A Personal Viewpoint
In my opinion learn to use the machine before you start adding bling. Accessories are nice and often useful but if you don't know how to use the machine in its basic form you likely will have more trouble than the accessories are worth.I have a buttonholer for my machine which I never use. I have no need for high quality specialized button holes. The "auto" feature on the machine works fine. That said.. If someone is doing outerwear and wants to include buttons on a jacket or some other heavy style of gear then there is no other way to get a "keyhole" buttonhole. (They have a round hole at the one end which allows the button to sit tightly and securely.) But that is pushing the "sewing" end of the spectrum.

I'll be honest, I don't know. Everything is included in the box and I'd be glad to take more pics for you. It looks like brand new. I got this with a sewing machine I bought on craigslist.
