    +1 for something under the neck...my favorite is my fleece jacket. It keeps my head tilted back and my airway open (actually same position used for opening the airway of an unconcious person in the medical setting) This also keeps my sonorous snoring to a hush.... I think.

    I use a very small backpacking pillow, by REI that stuffs inside itself. My large Thermarest is too big and gives me a neck ache. I also tie it inside to ridgeline near the gathered end, with a piece of cord, to keep it in place and from falling out on the ground

    I think whether one is a back sleeper or a side sleeper has a lot to do with one's preference. I'm totally a side sleeper and a pillow is a must. I use Montbell's inflatable pillow, only half inflated. That one is shaped, unlike most inflatables, so that my head doesn't role off in the night.

    I love the Cocoon Ultralight Air-Core Pillow. It allows me to inflate it to whatever size I need, plus it has two sides, one for warm nights and one for cold nights.
