Mine was a failure or ripped webbing strap on the Yampa River last summer. The whole camp heard it. It was slow rip and a thud and after they picked themselved up off the ground from laughing, they came to see if I was OK. I attributed it to sharp edges on the cinch buckles and possibly operator error by not making sure everything was alligned. However, I have since changed to descender rings.
