Mine was a failure or ripped webbing strap on the Yampa River last summer. The whole camp heard it. It was slow rip and a thud and after they picked themselved up off the ground from laughing, they came to see if I was OK. I attributed it to sharp edges on the cinch buckles and possibly operator error by not making sure everything was alligned. However, I have since changed to descender rings.
We are now ready to start our way down the Great Unknown. We are three quarters of a mile in the depth of the earth. We have an unknown distance yet to run, an unknow river yet to explore. What falls there are, we know not; what rocks beset the channel, we know not; what walls rise over the river, we know not. Ah, well! We may conjecture many things. The men talk as cheerfully as ever; jests are bandied about freely this morning; but to me the cheer is somber and the jests are ghastly. Powell 1869

I had a continuous loop made from a poorly tied double fisherman's knot fail. My butt hurt for like a month after that!

I had a sand anchor tear out of the ground and drop me in the middle of the night. I'm just glad I didn't get the 2x4 in the head.

Before that, I had a material failure once that dropped me right on my *** THROUGH the hammock. It was a cheap POS I got on my travels.

I thought I'd try out one of those cheapie key fob carabiners that sell for about buck near the cash register when you are waiting in line. Because I knew it was a "test", I made sure the ground under me was soft. It didn't break right away. It gave me time to get comfortable and relax first.

Poor rope selection (noob at the time). It snapped and I fell only about 12", but it pulled a muscle right near my spine that set off the worst back spasms I've ever felt. Missed 2 days of work because I couldn't move. Amsteel to the rescue!
