I use a mid-weight hooded zippered microfleece jacket. I roll it up and stuff it into its own hood. Works perfectly for me, and is multi purpose! Hooray for multi purpose gear!
I use a rolled up shirt most of the time. Don't need much, just a little neck support when I'm on my back.
Pillow...definitely. My posture doesn't tolerate going w/o one.
Use one from Walmart. Terry cover over thin layer of memory foam and a flat inflatable bladder. The bladder is actually dimpled in the center so your head doesn't roll off like it does with most inflatables.
Also have a FlexAir, but haven't tried it yet, for the reason mentioned above.

I find I need less pillow in a hammock than on the ground, but either way, it's clothes stuffed in a sack. I use a cotton stuff sack for this very purpose, since a silnylon bag just doesn't cut it, no matter what it's stuffed with.
The usual stuffing is my down vest.
I find that I need only a minimal pillow in my BB. Usually I just bunch up my jacket or other piece of clothing. I have a light inflatable pillow that I usually bring along on trips, but it goes under my knees to prevent hyper-extension.
I use a microbead pillow. It weighs 9 oz. and is extremely soft and comfortable. It packs up about the size of a down bag.
I left it at home once because of the weight. I won't again.

I use a packable pillow from Campmor. Also found it's comfortable to use my clothes bag like a pillow under my knees in order to give my legs a slight bend.
