Just to make sure I understand your original post, your inner shell is 46x60 and your insulation and outer shell are 48x60? What make you go that route vs. just a straight sandwich of layers?
You make the outer shell and insulation a little bigger than the inner so that when the UQ gets tightened into the U shape under you the outer layer doesn't compress the insulation. Basically looser fit when UQ is flat but it tightens up when formed into U. At least that what I understand.
Mostly. You want the insulation to be able to loft. Having the outside larger allows it to sag more than the inner layer and leaves room so the insulation doesn't compress. My reason for an Underquilt is so that I don't compress the insulation in a sleeping bag.
Remember compressed insulation doesn't work well.

Got it. How do you take up the slack in the differential? Some strategically placed folds?

I don't think you worry about it. Its not a lot and when its cinched up against the hammock the wider material is tighter. Not so tight that it compresses the insulation though.
