I carry one and sometimes use it, other times not, depending on how arthritic I feel. It's an inflatable neck pillow like you see folks using on aircraft. Before I got my sag dialed in, I sometimes needed one under my knees for leg hyperextension.

No pillow for me. Occasionally I'll use a small fleece throw under my head, but I don't feel like I need it.
I have a DIY pillow about the size of a jar of spaghetti sauce that is 'just right' to get my neck at a good angle when I get on my side in the hammock in the fetal position. When I back sleep it just becomes something to hold in my arms, or even a mobile piece of insulation in the case I need it to stuff in a cold spot.
These are some great responses guys. I will have to get the hammock slung up in the basement soon but I'm still cleaning up after the sump switch failed and flooded our basement with about 4" of water...I had to pull out all the carpet and move all of my man cave setup around to clean up. Should be ready for a test hang within the week...I'll have to try two seperate overnighters. One where I skip the pillow altogether and another night where I use a little bit smaller pillow. Even though my current pillow is just a small one, I'll have to try one that just supports my neck and see how that goes...I'll definitely keep you updated. Please keep sharing any thoughts or rationales on why you do or do not use a pillow.

I underinflate my Sea to Summit Premium Aeros and it works wonders.
I use a medium sized REI pillow. Now I am wondering if it is really just a comforter!?
I have neck issues, so, I started out using one of the neck pillows. It was way too thick. I used a stuff sack with minimal stuffing. It worked okay, but would get lost when I didn't need it. I've got the idea to make a "snake pillow", thin, not thick like the neck pillows, that you wrap around your neck and one end loops into the other end. Obviously, I don't wish to end up choking, so it would be designed to tighten only so far. But, the more I sleep in the hammock, the more I find using a pillow is not needed, and I've been amazed at the lack of neck sore after sleeping without pillow.

Forget the box, just think outside!
I start out with the pillow under my knees or knee. The hammock supports my head without a pillow. By morning, my body is all relaxed and I can roll on my side. In that case I might bring the nice warm pillow up to my head - maybe more to push any nylon out of the way. So if anything, it supports my head if on my side, but if I'm more on my back, it puts took much of an arch under my neck.
