It's not as big of a deal in a hammock, but on the ground the karo design can be harder to get right.
If you think of your insulation like a traditional sleeping bag (a full circle):
On the ground- the TQ wraps nearly 3/4 of your body in a relatively tight radius. Your pad covers the other 1/4.
In the air- your UQ covers 2/3-3/4 of your body in a roughly half circle shape. Your TQ covers the remainder but lays pretty flat over all.
The exception is something like a bridge- where the split is closer to 50/50.
You can certainly make a differential cut karo design. You can also go with a bit of overkill philosophy too (using a warmer quilt than needed to cover any issues or adding overstuff) to do the job.
But generally speaking when you need a more curved profile a more traditional baffle design works better up top.
For example if you are side sleeping- it's easy to find a cold spot develop on your shoulder/hip in a Karo step because the down will want to migrate away from those high points a bit.
Again though- as a back sleeper in a hammock you are probably fine either way.
Overall my comments come from a weight/efficiency background so they deserve a grain of salt if you're just looking to make one for yourself.
That said though... there is a reason why you see most, if not all manufacturers avoiding the karo design. Even Enlightened (Tim was one of the early proponents of Karo step quilts) only uses the design in it's UQ and has developed his shiftless baffle design for up top which is one of the best designs for a TQ anywhere.
Ok so after some time ive finally gotten around to putting together my under quilt. Fabric cutting went well. Laying out the pattern wasnt the easiest, ended up using tailors chalk and a marked square for a pattern but i got it ok in the end. The boxes were intimidating but not imposible to sew, just had to take my time and double check my baffles. Still some sewing to do but im moving right along finally.

Having filled a standard type baffles quilt now twice and once from a single 1 lb bag of 850 GP down - I feel much of the matter is hyped.
Get in shower - handle down in there use slow movements and just cram it in - wasn't that bad for me - wore a pair of shirts only and a buff over my nose and mouth.

Welp i got it all done minus the suspension. I am very very happy with it. Need to even out the down still a litte but thats not a problem. 12.3 oz as it sits.
