Never to all. But have always been extra careful in which knots I use for supporting my weight, and whether they are prone to slippage in Amsteel. Never have supported my weight on less than 1/8. BTW, for knots that don't break but might slip (not to be confused with those that capsize), a Figure of 8 or 9 or 10 stopper knot on the working ends can be a preventative for that. I've sometimes added those for extra insurance but I don't use knots that are known to slip in Amsteel anyway.
I can’t say that I have. The best part of amsteel is that you don’t have to tie knots in it. I did have a burry back out on a hammock ridge line but that was my fault since I had grabbed it and put my weight on it. I think it would have held if I had made the burry longer. I am a 375lb guy and I’ve never seen amsteel break.

7/64 has broken on me once with speed hooks/ hammock anchors. That kind-of counts as a knot, because the compression is what broke it.
I broke a 7/64 loop trying to cowboy rig my tato stand without the top bar. The line shredded against a sharp edge.

I stretched some 7/64 basically horizontal between 2 trees maybe 8' apart. (Like 12" off the ground), then stood and bounced on it (im 160 lbs) and it broke at the knot...was a long time ago and i dont recall the knot i used un fortunately.
From my testing using a force guage on the end of the hammock..if using a full dyneema suspension strung pretty tight...you can create force (at least) 240% of your bodyweight at each end. Specifically we created 530 lbs on one end of the hammock using a 225 lb person and a tight hang with a decent entry bounce. Assuming knots in dyneema break at 50% you could certainly break 1500 lb dyneema if you hang tight and/or are a heavier person.

To answer the question, no, I have not had any Amsteel failures. I routinely tie continuous loops with a single fisherman's bend, backed up with buries. No problems.

So far I have avoided tying knows on rope, all my knots are not webbing and try for knots that will minimize the webbing's strength. Marlin spike, and bowline on a bight are the two that I commonly use to setup my hammock.
