I have a camping pillow that is a small pillowcase with a thin lining of polyester on one side. You stuff some spare clothing in it and with the polyster-lined side up, you have a functional pillow.
I recently ordered a whoopie sling suspension from Paul, and added a Snoring Cub Pillow to my order. That little pillow is a work of art. I have a prostate that's bucking for honors in The Guiness Book of World Records, so I'm up two or three times during the night. The keeper shock cord at the corner of the Snoring Cub means I don't have to fumble around looking for my pillow upon my return. I just climb back in, reach up and cock the pillow sideways and it's just perfect. I wondered if it might be too small, but it really is a perfect pillow that I found adds to the night's comfort.
I use a small folded blanket as a pillow under my head. If I get cold during the night I put it over me, reach up and grab some other item (gloves, socks, balaclava…) from a stuff sack hung from the end of my hammock and place it under my head instead. Under my knees I have a lot of surplus clothes as a big pillow. Otherwise I tend to get cramps in my legs or I wake up in the middle of the night with hurting heels.
Using this setup I have the best sleep imaginable.

Since I'm fairly new to hanging (less than a year) and have only slept in the HH ULBA I can't speak to how I might like going without a pillow in another type of hammock. So far, I've tried without a pillow (it's ok I can do that if necessary), with a therm-a-rest fleece pillow cover stuffed with some clothes (I don't like it), a regular therm-a-rest pillow (too bulky) and finally my small Sea-to-summit pillow (I like). I'm thinking about adding a length of bundgy cord to the pillow so that I can connect it to the ridgeline like the pillows from Arrowhead. Even if I never do that though I find that it works fine the way it is.

But for the new folks, when I got into hanging I decided I HAD to have something or my neck was sore. I did not think I needed much but just a little something to support my neck through out the night. The Cub Pillow was born and now almost a year latter having started with that one product things have....well changed a bit for us. I vote pillow, take one every trip, well worth the 3 oz to me, I fell more comfortable and have the little bit of support that my neck wants so that I wake up with all of me feeling relaxed and without any stiffness.

Check this pillow out from Feathered Friends. It's a bit pricey, but it's the best pillow I've ever had. Just the right amount of support in the hammock. When I go to ground, I add additional height to it.
