Over the head for me. Always nice to have extra room to cover your head if'n necessary, or use the extra bunched up behind your head as a pillow. For the little extra weight, it's worth it.

Chin. In my experience, pulling the quilt to the top of my head allowed drafts if I wasn't very careful and I don't like the idea of breathing all that water vapor into my down. Plus, it weighs less.
P.S. - If you want to be able to use it on the ground also, think about making it 54" finished width. 50" leaves you very little margin for error when rolling around in your quilt.

Great question. Like others I recommend the over the head option. Also make sure to factor in the draw string cinched up factor as well.

I'll second the idea of ...
my summer TQ (50F) does not go over my head, but my 35F is long enough for complete coverage...
I prefer over the head a little as well. Just this weekend I slept in till almost 9 by pulling the quilt up to block out the sun. Did not really need to do that for warmth at all but that dang sun just comes up to early on my days off.
