Is there a free one somewhere around for synthetic insulation?

Check out this link - you'll find other MYOG stuff there, too. It's a very basic quilt without features, but it does have a footbox.

Having used a Speer Top Blanket, I go against the popular opinion and give a thumbs up to making a narrow TQ. The STB is 40" X 72", with velcro on the edges of the bottom 18" so it can make a footbox. When fully fluffed up, it's about 36" wide!

I too found that I had different needs between my summer down top quilt and my winter down top quilt. I like my summer quilt to be able to go totally flat. I've got a cord at the bottom with cord-locks on either side so I can gather up the bottom if I want to. My winter quilt is made with more of a foot-box but I can still undo it, it has velcro and ribbons that I can tie if I want the "foot-box" effect. Plus it's got cord and cord-locks to keep it gathered. I also put cord and cord-locks at the head end of the winter quilt so I can gather that up If I need to.
I've found that I really like the DIY quilts because I can change things to make the quilt to exactly how I want and need it to be. And I've found that after I've made the quilt in the pattern that I "thought" I wanted, if in testing I find that things are not working or I discover that I want to try another idea, I usually can just change it. I'm finding that I like my quilts to be more versaltal and having the cord, cord-locks, velcro, ties ect... makes the quilt able to function (at least for me) in a wider range of situations.
Good luck with your quilt making and then quilt testing!!!

I used the the BPL example and the KickAss quilt tutorial (help with sewing techniques more than shape), but it seems the KickAss quilt tutorial link no longer works
